Due to social distancing guidelines, all patients will be checked in while in the parking lot.  Upon arrival, please text your name and license plate number to 303-963-6163.  You will receive a call back to complete your symptom screen (you can also complete this on the day of your visit through your My Health Connections account).  Once your symptom screen is complete/verified, you will be asked to come up to the 2nd floor for your temperature check (all patients and visitors with a temperature of 100.4 or greater will be asked to reschedule their visit).  All patients and visitors will need to bring their own protective mask, this cannot be a bandana, neck gaiter/buff, or mask with a ventilator.  Thank you for your patience and understanding.
